Akorn promethazine with codeine dosage
2.1 Essential Dose and Administration Guidelines
Give the oral solution of promethazine HCl and codeine phosphate alone orally.
Always use an accurate milliliter measuring equipment while providing promethazine HCl and codeine phosphate oral solution to guarantee correct measurement and administration of the dose.
A teaspoon is not an accurate measurement tool and could result in an overdose.
For prescriptions that do not include a measuring equipment, a pharmacist can give a suitable measuring device and instructions for measuring the correct dose. Do not overfill. After each usage, rinse the measurement gadget with water.
Instruct patients not to increase the dose or dosing frequency of promethazine HCl and codeine phosphate oral solution since overdosing may result in serious adverse effects, including respiratory depression.
If cough fails to respond to promethazine HCl and codeine phosphate oral solution, the dosage should not be raised; instead, the underlying cause of the cough should be investigated.
2.2 Recommended Dosage
Adults 18 and older: 5 mL every 4 to 6 hours as needed, with a maximum of 6 doses (30 mL) per day.
2.3 Treatment Monitoring, Maintenance, and Discontinuation
Prescribe promethazine HCl and codeine phosphate oral solution for the shortest term consistent with the therapeutic objectives of the individual patient.
Watch patients closely for respiratory depression, especially during the first 24 to 72 hours of medication initiation.
Reevaluate patients with persistent cough within five days for potential underlying diseases, such as foreign body or lower respiratory illness.
If a patient needs a refill, examine the etiology of the cough and assess the need for continuous promethazine HCl and codeine phosphate oral solution treatment, the relative occurrence of adverse events, and the development of addiction, abuse, or misuse.
Do not cease promethazine HCl and codeine phosphate oral solution abruptly in patients with physical dependence.
When a patient who has been taking promethazine HCl and codeine phosphate oral solution consistently and who may be physically dependent no longer requires therapy, reduce the dose gradually, by 25% to 50% every 2 to 4 days, while closely monitoring for withdrawal symptoms.
If the patient develops these signs or symptoms, increase the dose to the prior level and taper more slowly by increasing the time between decreases, decreasing the amount of change, or both.
